Position Summary: The Program Navigator works as an integral member of the project team and provides comprehensive coordination of all care for program participants. The Program Navigator is a three-year grant-funded fulltime position that may be extended depending on future funding.
Duties
□ Create and implement a Re-Entry Plan for individuals in the Jail that
need outpatient follow up services.
□ Collaborate with our Community Law Enforcement Officer on follow ups
after overdose calls for service and other first responder referrals.
□ Prepare and obtain all relevant documentation to enable participants to
interact and talk with service providers.
□ Maintain a case file for each project participant including, but notlimited
to, a comprehensive care plan and re-entry plan.
□ Ensure that individuals have established links to appropriate careand
continue to use the services as recommended.
□ Ensure that participants have basic needs, such as transportation to
services, food, clothing, insurance, and housing.
□ Must establish and maintain successful working relationships withall
program partners and clients.
□ Maintain confidentiality of individual and family information asoutlined
in the framework of the law.
□ Provide case management for Drug Court Participants and attenddrug
court staffings and meetings.
□ Assist the Boone County Health Department with its Medication Assisted
Recovery Bridge Clinic, which may include conducting urine drug screens
and other clinic duties.
□ Participate in community meetings and subcommittees.
