Provide community support and case coordination services for a caseload of individuals with disabilities, serving primarily persons with developmental/intellectual disabilities.  Assess consumer’s overall functioning, identify problems, needs, skills strengths and deficits, and assume a lead role in the consumer centered planning process by developing implementation strategies.  Implements selected aspects of the plan including skill building, counseling, advocacy, and building community, family, and natural supports.  Ensures linkages to community service providers, entitlement programs and housing alternatives.

Must be able to communicate effectively both verbally and in writing, have good social and assessment skills, and ability to work both independently as well as part of a team.  Must have a commitment to work with persons with disabilities and demonstrate a belief in the principals of recovery and empowerment for persons with disabilities.  Willingness to work flexible hours and basic computer navigation and word processing skills essential.


Bachelor’s degree (B.A/B.S) from four-year college or university in the human services field and at least one year of direct care experience working with persons with disabilities.  Must be able to achieve Qualified Intellectual Disability Professional (QIDP) status to work with persons with developmental disabilities.  Must have Mental Health Professional (MPH) status to work with persons with mental illness.
