Medical Social Worker – BSN

Job Summary:  The social worker provides social work services to patients and their families in order to enhance psychosocial functioning and continuity of care. The social worker conducts psychosocial assessments of patients/families.  The social worker, in collaboration with the multidisciplinary team, develops and implements discharge planning for identified patients. The social worker provides psychosocial intervention with trauma victims, patients with psychosocial disorders, and patients having a difficult time adjusting to an illness.

Education/Experience:  Bachelors Degree or Master’s Degree in Social Work, Sociology, or Psychology plus experience in a health care setting.  Requires the ability to write information and to enter/retrieve information from a computer.  Work involves mobility throughout the hospital.  Exposure to blood-borne pathogens requiring the use of universal precautions.  Requires the ability to speak clearly and to hear conversation when there is background noise.
