Job Summary:  Under the direction of the Director and in collaboration with the Supportive Care Nurse Practitioner, the Nurse Navigator is responsible for overseeing, coordinating, and educating the FHN Supportive Care patient population. Triaging phone calls from patients and families, staff, providers, and other healthcare workers internally and externally and addressing their needs and concerns promptly to assure high quality care. Obtains information on patient referrals, reviews the medical record and communicates with the Supportive Care Nurse Practitioner to make an information visit and/or admit the patient or performs the info/admit directly. Coordinates patient clinic and home visits. Completes telephone follow-up on a routine basis to monitor patient status/needs. Communicates with attending physicians, various hospital staff members and other agencies as needed to coordinate care, obtain durable medical equipment and assist with prescription changes/new prescription orders coordination. Provides direct patient care as needed. Works collaboratively with all members of the interdisciplinary team, physicians, and other agencies.

Education/Experience:  Bachelor’s Degree in Nursing with 2-5 prior field-related and/or job related experience.  Current licensure as Registered Nurse in the State of Illinois.  BLS certification required.    Excellent computer skills, including clinical systems and data collection.  Demonstrate initiative and ability to function independently as well as interdependently.  PRIDE Behaviors.  Problem solving skills.
