Job Summary: This position supports the department by coordinating the case management services. Ensuring compliance with different case management programs and keeping quality of processes and documentation.
Provides time limited outpatient, clinical case management, and aids patient to improve functioning. Provides case management interventions to assure linkage to needed community resources to address all biopsychosocial needs.
Completes a thorough assessment and treatment planning services that secures intake and social history information such as medical, psychological, and social factors contributing to patient’s situation; and develops treatment plans in conjunction with the patients to address goals.
Assists patients with budgetary tasks and the management of personal finances.
Develops and review processes and documents needed for Case Management.
Completes reports needed for Case Management Programming (for example CCBYS).
Provides crisis evaluations, crisis intervention, and crisis case management to coordinate placement of patient to a more appropriate level of care. Develops deflection plans in coordination with patient supports, consultation with professionals or other resources, and which are appropriate for the level of risk. This including scheduling follow-up contacts for patients that are deflected.
Provides education and manages case managers, including rounding and HR needs.
Education/Experience: Bachelor’s Degree, must qualify for MHP status at a minimum. 5-8 years of experience preferred; supervisory skills clinical knowledge of standard and acceptable case management interventions for the mentally ill or dually diagnosed individuals; knowledge of risks and protective factors regarding harm and self-neglect due to behavioral health concerns; knowledge of basic Medicare/Medicaid/Joint Commission standards and guidelines; ability to make informed decisions about patients to ensure quality services; knowledge of Case Management Program Requirements.
