Position Summary: The transition of care pharmacist plays a critical role in the patient discharge by ensuring a smooth transition between hospital and home or skilled placement. Their primary responsibilities included medication reconciliation, medication regimen optimization, counseling and education, as well as collaborating with other members of the healthcare team, caregivers and the patient to coordinate care plans. They will work to review patient medications and medical history to identify and resolve medication related issues. High level communication and problem solving skills in combination with a commitment to patient advocacy are needed for this role. This position will collaborate with the pharmacy staff and participate in the non-transition of care tasks and support as needed and appropriate.
Education/Experience: 1-2 years prior field-related experience with patient counseling experience preferred. A degree as a PharmD is required.
