Job Summary:  The Complex Care Manager (CCM) is a RN who is responsible for the Care Coordination and Transition Management of patients throughout the care continuum. The CCM requires the ability to communicate effectively with patients, care givers, members of the healthcare team and outside agencies. The CCM fosters inter-professional collaboration, evidenced based delivery of care, and complex clinical and regulatory knowledge. The CCM promotes patient engagement and individualized care planning with patient specific goals. The CCM collaborates with the health care team to avoid duplication and fragmentation of resources while promoting optimal outcomes of the patient. The CCM is able to work autonomously within the domain of nursing to identify and access community resources that meets the individual patient needs. The CCM will participate in data collection, monitor trends, and contribute initiatives to improve patient care and the complex care management program.

The CCM will collaborate with the healthcare team to identify patients who are a high risk for complex care management. The CCM will follow the standards including assessment, care planning, goal setting, implementation/coordination of care, and evaluation.  The CCM will report to the Director of Resource Management and will collaborate with the FHN leadership team to ensure alignment with FHN strategic goals, mission and vision. The CCM will adhere to FHN PRIDE standards.
Education/Experience:  An Associate Degree in Nursing is required with BSN preferred.  3-5 years prior field-related and other job-related experience highly preferred.  Certification in complex care management preferred.  Valid driver’s license and proof of vehicle insurance. required.
